Promoting Self Management and Recovery Tools and Technology

  • Expand the scope of practice for the Wellness Management and Recovery CCOE to provide technical assistance in health promotion and disease prevention.
  • Provide workforce development for clinicians beyond utilizing treatment and medication as the mechanisms to foster recovery and provide skills and resources to help consumers in Ohio achieve a better quality of life. Seventy percent of health outcomes are accounted for by environmental and behavioral factors.
  • Increased emphasis on person-centered care and shift the burden of practitioners from symptom monitoring and medication compliance to promoting individuals and families in self-managing long-term conditions. People are much more likely to follow through with treatments they have chosen and less likely to follow through on treatments chosen for them.
  • Increasing the confidence, self-efficacy, and activation among persons living with SPMI by offering educational resources and practical decision making tools in Wellness Management and Recovery and Chronic Disease Self Management Program (Stanford University). These programs help to increase awareness and knowledge about health and wellbeing thereby improving patient activation, informed decision making and wellness planning.
  • Contain costs – Self Management programs help direct people toward more appropriate levels of service and away from higher or more costly services (Lorig, 1999). Less utilization of emergency rooms and frequent hospitalizations.
  • Peer Support – when care is self directed, individuals are more likely to seek out peers who share similar experiences and challenges. Peers provide ongoing support for achieving health and wellness goals.

CDSM helps persons with SPMI to either prevent the onset of chronic physical health conditions or to learn how to self-manage physical healthcare conditions (i.e. Diabetes, Asthma, Heart Disease). Conditions which may be attributed to psychiatric care (ex. medications)
